Home and Personal Support

• Personal care services and homemaker services for members who need assistance with daily activities to remain safely in their homes
• Environmental accessibility adaptations such as grab bars, ramps, and other modifications that allow members to live safely at home

Other Supports

• Asthma remediation, which addresses environmental factors in the home that contribute to asthma
• Sobering centers for members who need a safe place to sober up without requiring emergency room care
• Day habilitation programs for members with disabilities

Who Qualifies for Community Supports

Community Supports are available to Medi-Cal members who are enrolled in a Medi-Cal managed care plan, which covers the majority of Medi-Cal members in California including those enrolled through Partnership HealthPlan and Kaiser in Solano County.

The Difference Between Community Supports and Enhanced Care Management

Community Supports and Enhanced Care Management are both part of CalAIM, and they often work together, but they are not the same thing.

Housing navigation helps members who are experiencing homelessness or housing instability identify appropriate housing options, complete applications, and navigate the process of securing stable housing.
