At Solano Impact Care, we understand the urgency of your situation because many of us have been there ourselves. We provide dedicated support for mothers in crisis, offering a path to stability, safety, and hope.

We know that when you’re in survival mode, the last thing you need is a confusing system to navigate. You need a hand to hold, a safe place to land, and a team that will fight for you and your family. We are here to provide comprehensive support to mothers facing immense challenges. You may qualify for our specialized programs if you are a Medi-Cal member in Solano County (through Partnership HealthPlan or Kaiser) and are dealing with any of the following situations:

One of our cornerstone programs is Hazel’s Tranquility Place, a dedicated home for women, including mothers with their children, who need a safe and supportive living environment. This is more than just a shelter; it’s a community where you can heal and rebuild.

This model is the heart of the CalAIM initiative, a new approach in California to providing care for Medi-Cal members. CalAIM (California Advancing and Innovating Medi-Cal) is built on the understanding that real health and stability come from treating the whole person, not just their medical conditions. Enhanced Care Management (ECM) and Community Supports are the key services under CalAIM that allow us to provide this intensive, personalized level of care.

We understand the urgency of your situation. While we are not an emergency shelter, we can connect you with emergency housing resources and work with you to find a long-term, stable solution, such as women’s transitional housing in Solano County or a spot at Hazel’s Tranquility Place for women.

Yes. Family reunification is a core part of our mission. We provide the support and advocacy needed to help you navigate the systems and meet the requirements to bring your family back together. We offer dedicated family support services in Solano to help you on this journey.

We offer comprehensive mothers homelessness support. This includes helping you find safe, temporary housing, and then working with you to secure a permanent home. But it’s more than that. We help you address the root causes of your housing instability, whether that’s a need for employment, childcare, or healthcare.
