They are more likely to have unmanaged chronic conditions because the basic requirements of consistent medication, proper nutrition, and adequate rest are impossible to meet without a stable place to live. They are more likely to experience mental health crises because homelessness is itself a traumatic and chronically stressful experience. They are more likely to cycle through emergency rooms and hospitals because they have no place to recover from illness or injury. And they are significantly more expensive to serve through the healthcare system, because the cost of repeated emergency interventions far exceeds the cost of stable housing and preventive support.
California’s CalAIM reform, launched in 2022, formalized what advocates and community organizations had been saying for years: you cannot improve health outcomes without addressing housing. The program created a set of housing-related services that Medi-Cal managed care plans can now cover for eligible members, funded as healthcare because housing instability is a healthcare problem.
What Medi-Cal Can Actually Help With
The housing services available through Medi-Cal fall under the CalAIM Community Supports program. They are not a single service but a set of distinct supports that address different housing needs at different stages.
Housing navigation
This is active, hands-on help finding housing. A housing navigator works alongside you to understand your situation, identify the housing options that are realistic for your circumstances, help you complete applications, gather required documents, and advocate on your behalf with landlords and housing programs. Move-in assistance
One of the most common barriers to housing for people with very low incomes is the upfront cost. First month’s rent, security deposits, and the practical expenses of moving in can add up to thousands of dollars that many people simply do not have. Move-in assistance through Medi-Cal Community Supports can help cover those costs for eligible members.
Housing tenancy and sustainability services
Getting housed is the first step. Staying housed is the ongoing work. Sustainability services help members who are already housed maintain that housing by providing support with understanding lease obligations, communicating with landlords, accessing emergency assistance when rent is at risk, and resolving issues before they escalate to eviction.
Short-term post-hospitalization housing
When a Medi-Cal member who is experiencing homelessness is discharged from a hospital, they need somewhere safe to go to recover. Recuperative care

All of these services are provided at no cost to eligible Medi-Cal members through CalAIM’s Community Supports program. You do not pay a copay or a fee to receive them.
Who Qualifies for Medi-Cal Housing Help
These housing services are available to Medi-Cal members who are enrolled in a Medi-Cal managed care plan and who are experiencing or at risk of homelessness. This includes people who are:
- Currently unsheltered or living in a place not meant for human habitation such as a car, a tent, or an abandoned building
- Staying in an emergency shelter or transitional housing program
- At imminent risk of losing their current housing due to eviction, nonrenewal of a lease, or a domestic violence situation
- Exiting a hospital, jail, correctional facility, or treatment program with no stable housing to return to
- Staying temporarily with friends or family due to housing instability and at risk of losing that arrangement
In Solano County, Medi-Cal managed care plans include Partnership HealthPlan and Kaiser. Members enrolled through either plan who meet the housing eligibility criteria may be able to access Community Supports housing services.

The Role of Enhanced Care Management in Housing Access
For Medi-Cal members with the most complex needs, housing support through Community Supports works alongside Enhanced Care Management to address housing as part of a broader, coordinated care plan.

When housing is a priority in a member’s care plan, the care manager coordinates the connection to housing navigation services, stays involved through the process of getting housed, and works to make sure that housing is stable and sustainable as part of the member’s overall wellbeing.
This coordination matters because housing instability almost never exists in isolation. It is connected to mental health challenges, substance use, incarceration history, lack of income, family crisis, or some combination of all of these. Addressing housing as a standalone issue, without addressing the connected factors that contributed to the instability, tends to produce housing that does not hold.
The integrated approach, Enhanced Care Management coordinating across everything while Community Supports housing services address the housing piece specifically, produces more durable outcomes because it treats the whole person rather than one symptom.
What Medi-Cal Cannot Do for Housing
It is important to be honest about the limits of what Medi-Cal’s housing services cover, so that people have accurate expectations and can plan accordingly.
Medi-Cal does not pay rent directly or provide long-term rental subsidies in the way that programs like Section 8 housing vouchers do. The housing services available through CalAIM are navigational and transitional in nature. They help people find and access housing, cover short-term costs, and maintain stability once housed. They are not a substitute for affordable housing itself, which remains a serious and unresolved challenge across California including Solano County.
